Consent to personal data processing

I hereby, in accordance with Russian Federal Law No. 152-FZ "On Personal Data" dated 27.07.2006, freely, of own free will and violition give my unconditional consent to my personal data processing by Sespel Cheboksary enterprise, CJSC (OGRN: 1022101277934, INN: 2126002786 ), registered in accordance with the legislation of the Russian Federation at the address:
Chuvash Republic, Cheboksary, Leningradskaya, 36, space 5 (hereinafter referred to as the Operator).
Personal data is any information related to a certain person or defined on the basis of such information.
This consent has been given by me to process the following personal data:
Name, Surname, Patronymic, Phone, E-mail.

The consent is given to the Operator to perform the following actions with my personal data using automation technology and / or without using such tools: collection, systematization, accumulation, storage, clarification (updating, modification), use, depersonalization, as well as any other actions provided for current legislation of the Russian Federation both by non-automated and automated methods.
This consent is given to the Operator for the processing of my personal data for the following purposes:
- providing me with services / work;
- sending me notifications related to the rendered services / work;
- preparation and sending of answers to my inquiries;
- sending me information, including advertising, about the activities / goods / services / work of the Operator.

This consent is valid until it is revoked by sending an e-mail to: zaosespel@yandex.ru In case of my withdrawal of consent to personal data processing, the Operator has the right to continue processing personal data without my consent in the presence of grounds specified in clauses 2 to 11 of Part 1 of Article 6, Part 2 of Article 10 and Part 2 of Article 11 of Russian Federal Law No. 152-FZ "On personal data " dated 27.06.2006.
